Pick your number
The academy's registration form, online. Fill it in once for each player, choose a shirt number from the ones still free in their age group, and sign at the bottom, just as you would on paper.

Before you start
A passport photo
A clear, recent photo of the player's face. A phone photo against a plain wall is fine.
Birth certificate or passport
The number goes on the form. Bring the original when you first come to training.
Guardian and emergency contacts
Phone numbers for the parent or guardian, and for someone else we can call if they can't be reached.
Medical notes
Any conditions, allergies or medication the coaches need to know about on the pitch.

Who sees it
The form goes to the academy office and the coaches, and nowhere else. Medical notes, the date of birth and the photo are never shown on the site. The number board only ever shows numbers, never who has them.
